Purple/Alumni/Community Membership
Links to the Purple/Alumni/Community memebrship applications can be found below. These are fillable forms for your convenience, but must be turned into the Rec Center Office in one of the following ways. Payment cannot be made online.
- Mail or drop off your completed forms, along with payment to the Recreation & Wellness Office.
- Email your completed forms to recwell@ashland.edu and call the Recreation & Wellness Office at 419.289.5440, option 3 (Membership Inquiries) with your credit card information.

Gold Membership information (membership application, liability form, and FAQ's for Regular Full-Time and Regular Part-Time Employees) can be found on the Employee Portal > FS Info > Employee Wellness. Payment for this membership may be made online, if paying by payroll deduction.
