Privacy of Care

Protecting Your Personal Health Information (PHI)

The Student Health Center takes the security of your personal health information very seriously and has established strict standards and procedures to prevent unauthorized access to your PHI. Only authorized personnel within the Student Health Center who are directly involved in your care may access your PHI. These individuals are trained to handle confidential information and must abide by the terms of our confidentiality agreement.

Your Rights with Respect to Your PHI

In consideration of the law and your personal security regarding your PHI, you have the right to:

• Request restrictions on the usage or disclosure of your PHI
• Receive confidential communications of your PHI
• Inspect and copy your PHI. Note that the Student Health Center has the right to charge a reasonable fee based on the costs of copying, posting and preparing the documentation as agreed upon in advance
• Amend your PHI
• Receive an account of disclosures of your PHI

Information We Keep

The Student Health Center keeps the following information (including PHI) on file:

• Information you provide during a Student Health Center visit
• Treatment you receive during a Student Health Center visit 
• Immunization records you provide to the Student Health Center 
• Information we receive from other healthcare providers

Information We May Disclose

The Student Health Center does not disclose any PHI or personal, non-public information about students or former students to anyone, with two exceptions:

• With your written authorization we will, when necessary, share the minimum amount of information required for any person(s) or healthcare provider(s) you designate.
• We will share PHI under court order as required by law.
